  • Responsibilities concerning security or occupational health and safety issues of the employee (may include workplace or faculty information).

Occurrence reports:

  • Name of the employee/student/visitor (First name and last name)
  • Email address of the employee/student/visitor
  • Phone number of the employee/student/visitor
  • Faculty of the employee/student
  • Description of the accident or near miss situation happened to the employee/student/visitor
